COMMUNITY HEALTH PROGRAM

Mobile integrated healthcare - community paramedicine (MIH-CP) is mobile, patient-centered healthcare delivered in the out-of-hospital environment. Services are often provided in a residential location, but we offer flexibility to meet the individual needs of our patients. Our MIH-CP teams are comprised of Registered Nurse Case Managers and Emergency Medical Technicians working with Community Paramedics.

Our MIH-CP Programs are designed to provide our patients the following community health:

 

ASSESSMENT AND CARE

We'll perform assessments to learn about your history and determine your current health status. 

 

GOAL SETTING

Our teams work with each patient to set individual goals that we work together to achieve.

 EDUCATION & TRAINING

MIH-CP teams ensure you stay healthy by providing injury & illness prevention education.

SUBSTANCE USE DISORDER

Substance use disorder is a disease. Whether opioids, alcohol, or other substances are being used, we work closely with community partners to coordinate recovery that is holistic.
